Job Description:

A company is looking for an Acute Care Registered Nurse, Spanish Bi-lingual.

Key Responsibilities:

Provide patient outreach, triage, and disease education through telehealth modalities
Monitor chronic conditions and perform follow-ups with patients
Document patient encounters and participate in clinical case conferences

Required Qualifications:

Active, unencumbered Nursing license in state of practice
Bi-lingual in Spanish
Experience in care of adult, chronically ill patients
Working knowledge of electronic documentation systems
Availability for evening and weekend shifts
